I’ve got to say that throughout the past 2 years, I have gotten to love this app more and more. Can’t imagine life without it. Has cut down on phone calls to the level that I get surprised when someone calls me now.  The concept of texting patients is so obvious, yet no one thought of implementation until ERAS was needed.  Keep up the great work you are doing for patients and healthcare.

Patient had a lap right hemicolectomy. Went to outside rehab in 3 days. Daughter Twistled me that she had a fever last night. I ordered chest X-ray, urine, and bloodwork. Found to have early pneumonia. Started on abx.  Was not readmitted. Being managed at SNF. Doing fine. SNF didn’t notify me about any of this. It was managed early because daughter contacted us thru Twistle and we were proactive on a Sunday.

Patient Twistled me Saturday that he had a low-grade fever. Felt fine except for some mild right sided pain. I sent him to ER. CBC elevated. I ordered CT. Found to have small early abscess.  I admitted for IV antibiotics. Today he feels great and should go home soon. He was discharged 2 days after surgery. It shows we can early discharge and maintain good care with surveillance.

Another great save. Patient Twistled that she was having anal pain. I reversed her ileostomy about 4 weeks ago. She developed a Crohn’s perianal abscess which she has had in the past. This is unrelated to her recent surgery but would have made her sick if left untreated. I am going to drain in OR tomorrow as outpatient procedure.

Patient Twistled me that she was feeling weak and nauseated… I found that she was having frequent bowel movements from the surgery and was managing her frequent stools by eating and drinking less. I sent visiting nurse to check on her. Nurse placed IV and gave 2 liters NS. Nurse will continue to give 2 liters NS daily for the next 3 days. The patient will update me thru Twistle in three days to see how she is doing. Patient dehydration managed at home, no readmission.

This is a good save. Patient was having wicked diarrhea and was getting dehydrated. She didn’t have home health set up. I called home health, had them start an IV on her and had her get 2 liters IV fluids over this weekend. She feels better and diarrhea resolving. In the old days, this would have been 100% a readmission.
